What to Bring to Your First Visit
A little preparation helps your first appointment go smoothly. When you contact Glacial Dental to book, we will let you know what we need. In general, it helps to bring:
- A photo ID
- Your dental insurance card, if you have coverage
- A list of any medications you take
- Any recent dental X-rays or records you already have
We will also ask you to fill out a medical history form. This helps us understand your health and plan care that is safe for you.
Arriving a few minutes early gives you time to settle in. If you are running late or need to reschedule, just give us a call at 216-351-5500.
And if you are unsure about anything, ask. There are no silly questions here, whether it is about a treatment, a bill, or how long your visit will take.
